If scheduling for a minor, please list their name and age. If scheduling for a child over the age of 18, please list name, age, and their contact information as well.

Cancellation Policy

Cancellations must occur within 24 hours prior to your scheduled appointment. We do charge a full session fee ($100) if you do not cancel within this time frame or are a no call-no show. Insurance does not cover the cancellation fee. We understand emergencies happen and are willing to work with you in these cases in which rescheduling within 7 days is recommended.
